Fluid level in the heart!!

An old lady has been recently admitted to our medical ward, in Benghazi Medical Center.
The reason she presented was increasing fatigue and dizziness.  She had no recent bleeding, or NSAIDs use.
She was pale, and thin, old (75 years) lady, whose past medical history is remarkable for chronic anemia.
Systemic examination revealed nothing abnormal.
Her Hb was 3 g/dl (it was 7 g/dl, few months back), with low serum iron and ferritin levels.
ECG was normal, and plain chest radiograph showed a fluid level across the largest diameter of the cardiac shadow.
